We know that you care about your patients and if you are recommending Ketamine therapy, your patient may be struggling.  We strive to make the onboarding process for new patients quick and easy so that we can quickly get to the business of making them feel better. Here, you will find a patient referral form that patients, physicians, and other ketamine providers can easily fill out and send to our office.

The quickest way for a patient to get started with us is for you to provide us with a patient referral.  This provides us with the confirmation of diagnosis needed to move forward and book the patient's therapy appointments.  All you need to do is send over a quick fax or secure email with the patient's information and diagnostic codes.  Often, this is the thing that takes the longest for us to obtain and delays the process of getting patients started on the road to recovery.  Your referall speeds that process up significantly.

Our location in sunny SE Florida means that we see many patients who live here part time.  We work with ketamine clinics all over the country to streamline patient care so that your patient is able to continue care while they are here.  All we need from your office is the chart notes from your patient's last visit before coming to Florida.  When your patient is returning home, we will provide the patient with our last chart notes in their patient portal account, and/or fax them to your office.
